What is ‘Bridging The Gap’?
WHAT TO EXPECT
‘Bridging The Gap’ is an innovative project that explores how green therapy can help people transition from a hospital environment, back into the community, by bringing community services closer to them. It provides opportunities both on and off the wards to engage people in activities which can help reduce anxiety and depression, improve cognitive functioning, improve mood, reduce stress and anger, build confidence and self-esteem, help people be more active, make new connections, volunteer, learn / share skills, provide peer support and help people access training and courses that support recovery. Bridging the Gap aims to help people transition back into the community after their discharge by linking them with local groups to help them build social support networks – reducing health inequalities and building more resilient communities in areas of deprivation.
